Death certificate and certificate
The file usually begins with the death certificate or certificate. If the cause is not stated in that document, the authority may also request a medical certificate.
Health documentation for coffins
To transport a corpse, a certificate of embalming, proof of conditioning and sealing of the coffin, and a certificate of absence of infectious or endemic diseases may be required.
Necropsy and special authorizations
When the death was sudden, violent or subject to investigation, an autopsy certificate or protocol and judicial or fiscal authorization may be requested before cremation or transfer.
Mortuary passport and entry authorization
To transport remains or ashes to Italy, the consular representation may previously request authorization from the Commune of destination and issue a mortuary passport. Peru also requires health and legal documentation for the entry of remains from abroad.
Family and carrier documents
The file may include identification documents, proof of relationship, family authorization, power of attorney, companion information, air waybill, and details of the funeral home, flight, and destination cemetery.
A different file for each transfer
The requirements change depending on origin, destination, nationality, cause of death and method of transfer. Before translating, request the current list from the consulate, health authority, Commune, cemetery, funeral home and airline involved.
